The landscape of migraine prophylactic therapies has been reshaped by the recent emergence and regulatory approval of monoclonal antibodies that focus on the calcitonin gene-related peptide (CGRP) pathway. With the advent of novel therapies, leading headache societies have established protocols for their introduction and progressive use in treatment. However, there is a shortage of compelling data regarding the length of time prophylaxis is successful and the ramifications of ceasing the treatment. This narrative review examines the rationale behind the cessation of prophylactic therapy, integrating both biological and clinical aspects to support informed clinical decisions.
Three unique literary search methods were utilized for this narrative review study. Preventive treatments for migraine, including those for overlapping conditions like depression and epilepsy, are subject to defined cessation criteria. Furthermore, discontinuation guidelines for oral therapies and botulinum toxin injections are also established. In addition, protocols are in place for stopping treatments using antibodies aimed at the CGRP receptor. The following databases—Embase, Medline ALL, Web of Science Core collection, Cochrane Central Register of Controlled Trials, and Google Scholar—incorporated keywords for the search.

We examined if variations in ploidy levels cause alterations in sexual development and gene expression within the eri silkmoth, Samia cynthia ricini (2n=27/28, Z0/ZZ). Following heat and cold shock treatments, tetraploid males (4n=56, ZZZZ) and females (4n=54, ZZ) were obtained; these tetraploids were then crossed with diploids to produce triploid embryos. Triploid embryos displayed two distinct karyotypes, 3n=42 (ZZZ) and 3n=41 (ZZ). In triploid embryos having three Z chromosomes, the S. cynthia doublesex (Scdsx) gene displayed a male-specific splicing pattern; conversely, triploid embryos possessing two Z chromosomes showed splicing characteristics of both male and female variants. Despite their normal male phenotype, three-Z triploids, progressing from larva to adulthood, encountered defects in spermatogenesis. Anomalies were observed in the gonads of two-Z triploid individuals, where both male- and female-specific Scdsx transcripts were detected, not just in the gonadal regions, but also throughout the somatic tissues. Therefore, the presence of two-Z triploids clearly indicated intersexuality, suggesting that the sexual maturation in S. c. ricini is determined by the ZA ratio, and not the Z count alone. Finally, embryonic mRNA-sequencing experiments showcased that relative gene expression levels were consistent across samples with diverse Z-chromosome and autosomal set sizes.
